People with long term health conditions or social care needs have told us that they would like more information to help them manage their condition better and live longer, healthier and more independent lives.
Information could include details about conditions like asthma and diabetes; health & social care services; financial & housing advice; support groups; information for carers; education & training opportunities; employment & volunteering and more.
I've been given an information prescription - What next?
Your prescription will have one or more ticks next to the information categories listed on the left hand side of this website. Click on the categories and then search for the information you have been prescribed by looking at the list of topics in the blue box at the top of the page.
I'm finding it difficult to find the information I need.
You can get help by:
1) Visiting your local library - a member of staff will be on hand to help you; or
